The tonsils are a pair of distinctive cortical lobules that take in the most anterior/medial aspect of the posterior cerebellar hemispheres, just lateral to the inferior end of the vermis. They are clinically significant in that excess pressure in the posterior cranial fossa can push the tonsils into the foramen magnum (tonsillar herniation)and thereby compress the medulla and cause cardiorespiratory arrest.
